The documentation below is used by officials to administer games and file reports for various game-related events.
Youth Lacrosse Certification Message
At the beginning of every youth contest, the chief referee is required to make the following statement to both head coaches prior to the opening lineup: "Coaches please certify that your players are properly and legally equipped and know how to user their equipment. Additionally, please certify that only those coaches who are PCA certified will be allowed in the coaches area during the contest."
PIAA Sportsmanship & Certification Message
At the beginning of every PIAA contest, the chief referee is required to make the following statement to both head coaches prior to the opening lineup: "PIAA requires all registered sports' officials to enforce the sportsmanship rules for coaches and contestants. Actions meant to demean opposing players, teams, spectators and officials are not in the highest ideals of interscholastic education and will not be tolerated. Let today's contest reflect mututal respect. Coaches please certify to the referee that your players are legally equipped and uniformed according to the NFHS rules. Good luck in today's contest."
PIAA Sport Disqualification
During the PIAA boys lacrosse season, a disqualified lacrosse player, coach and/or team personnel shall not be permitted to participate in the next contest at the same level in accordance with Article XIII, Section 8 of the PIAA By-Laws. The official who disqualified the competitor and file this form as soon as possible, but no later than 24 hours following the lacrosse game in which the disqualification took place by forwarding it to the above address. PIAA Injury Report Form
If an injury occurs in a contest, PIAA is requesting one the contest officials to complete this report within 24 hours after the completion of the contest. The PIAA is attempting to document any and all serious injuries that occur during the interscholastic season. Submit PIAA Injury Report Form Online
